Goal six involves a prevention plan. Clients learn to identify situations that may lead to poor choices and learn to apply strategies for dealing with these situations. A checklist is a part of this activity so that it provides a step-by-step process for addressing these situations.
During aftercare, a client meets with the aftercare team and develops a plan for next steps in the program. Finding meetings in the area, locating the best option for living locations, and building a support network are part of the aftercare process. This includes locating friends and family, identifying social services available, finding work, and adding to a person’s education.
During graduation, clients invite family and special friends to share in the celebration of the program’s completion. Clients receive a certificate for successfully completing the program in full.
This step involves the client being added to a call list. The client is called each day for two weeks, then once a week for one month. After the month is up, calls are made once a month for one year. During this time, the aftercare contact person determines if the client needs additional support to successfully stay drug and alcohol free. Support is provided, as needed to ensure client’s success.
